As the final post for this case series, I wanted to share an upper first molar I completed last week. This young man had a deep restoration placed on tooth #14 five years ago. Unfortunately, the tooth abscessed on him recently. The case was completed over two visits, with Calcium hydroxide dressing used in between visits.
Diagnosis tooth #14: Necrotic pulp, Acute Apical Abscess
Looking at this case, I can find a couple of things of which to be critical. With that said, if you assess the postoperative radiograph, the communication between the mesiobuccal canals is quite evident. I have added a table of canal configurations created by Vertucci. Defining the configuration you are treating is a big help in successfully treating these cases.
Happy filing!
